Primary Care Provider – Mobile & Geriatric Medicine

$150,000 Base Salary + Performance Bonus | Monday–Friday | Phoenix Metro, AZ

Billet Health is looking for an experienced NP, PA, MD, or DO to join our growing Mobile Primary Care team serving older adults throughout the Phoenix metropolitan area.

This is an opportunity for a provider who wants clinical autonomy, meaningful patient relationships, and the freedom to care for patients where they live—without the traditional high-volume primary care environment.

You'll spend your day caring for patients in their homes and residential communities, building relationships, managing complex conditions, and making a meaningful difference in the lives of older adults.

Your Day Looks Different Here

Instead of seeing patients for 15 minutes in a crowded office, you'll provide care where your patients actually live.

You may see patients in:

• Private residences
• Assisted living communities
• Skilled nursing facilities
• Memory care communities
• Group homes
• Retirement communities

Your role will include:

• Performing comprehensive evaluations and physical examinations
• Diagnosing and treating acute and chronic conditions
• Developing individualized treatment plans
• Managing medications and prescribing appropriately
• Reviewing labs, imaging, and diagnostic results
• Managing chronic conditions and complex geriatric patients
• Coordinating care with patients, families, caregivers, facilities, and healthcare teams
• Providing preventive and transitional care
• Performing minor procedures and wound care when appropriate
• Completing timely and accurate clinical documentation

What We're Looking For

We're looking for a provider who is clinically confident, compassionate, independent, and genuinely enjoys caring for older adults.

Requirements:

• Active Arizona license as an NP, PA, MD, or DO
• DEA license with prescribing authority
• Board certification where applicable
• 1+ year of clinical experience preferred
• Strong clinical judgment and decision-making skills
• Ability to practice independently
• Excellent communication and patient relationship skills
• Comfortable providing care in community and residential settings
• Willingness to travel within an assigned Phoenix Metro territory

Preferred Experience:

• Geriatrics
• Family Medicine
• Internal Medicine
• Skilled Nursing / Post-Acute Care
• Home-Based Primary Care
• Complex Care Management
• Wound care experience is a strong plus

You'll serve patients throughout an assigned local territory within the Phoenix Metro area, including communities such as:

Phoenix • Scottsdale • Tempe • Mesa • Chandler • Gilbert • Glendale • Peoria • Sun City • Surprise
