Participant Care Plan Coordinator

Job Summary Sunrise Services, Inc. is seeking a Participant Care Plan Coordinator for our Supported Living Program. Base Pay: $26.00 - $31.00 / Hour Employee Type: F/T Non-Exempt 160+ Contact: View email address on click.appcast.io General Responsibilities Manage behavioral instruction and support services for participants with developmental disabilities. Manage staff and ensure effectiveness of client services. Promote dignity, respect, and a high quality of life for participants. Ensure that all participants receive care, support, and assistance in accordance with agency, local, state, and federal guidelines. Essential Functions Model a positive, proactive, and priority-based communication style that encourages collaboration and allows space for all voices to be heard, with the goal of improving the quality of life of our team and our clients. Participate in specialized training and other professional development activities designed to update knowledge, enhance and increase skills, and foster professional growth. Maintain knowledge of and ensure compliance with applicable Federal and Washington statutes and administrative rules, contracts with funding agencies, and company policies. Develop and implement appropriate records, tracking mechanisms, reports, and documentation, including tracking tools for staff to use in data collection. Author Functional Assessments (FA), Positive Behavior Support Plans (PBSP), and the Individual Instruction Support Plan (IISP) for adults in the Supported Living (SL), Community Protection (CP), and Specialty Program (SP) Programs. Conduct quarterly, bi‑annually, and yearly reviews of all FA/PBSPs and communicate the need for bi‑annual IISP reviews to ensure compliance for SL, CP, and SP. Maintain and update ISP data on Therap as required for client changes in support. Train and develop staff to implement and effectively manage the Positive Behavior Support Plan. Review data and author reports for all restrictive procedures and exceptions to policy (ETP) for SL, CP, and SP. Obtain necessary signature(s) required to ensure compliance for all participants, PBSP, IISP, and RP/ETP’s. Participate in weekly mandatory meetings with BPM (Behavior Plans Manager) and provide data that ensures plan compliance and general client updates regarding their support needs. Work directly with clients of all support needs, including but not limited to challenging behavioral support, mental health needs, sexualized behavioral support, and medically fragile support needs. Attend transition meetings as assigned by BPM. Author plans for new transitioning clients. Attend weekly meetings assigned by BMP to get to know new transitioning clients. Communicate and consult with DDCS Psychology Associates and Case Managers regarding clients currently in service as well as transition clients. Travel to meet with transitioning clients at their homes, RHC’s, and Department of Corrections facilities. Work directly in the SL, CP, and SP home(s), including direct observation of the client’s behaviors for data‑tracking purposes, training staff in the implementation of behavior and support plans, and consulting with guardians and outside stakeholders. Ensure prompt response and collaboration with QA and Health Care teams. Participate in on‑call rotation. Work direct care shifts during on‑call week whenever necessary to ensure required or appropriate staffing. While providing direct support for the clients in their homes or during weekly site visits, ensure the homes are clean and compliant per state requirements. Perform occasional light lifting of up to 20 lbs, plus lift and transfer clients as needed. Navigate stairs in multistory buildings without elevators. Observe the client’s functional conditions and report changes immediately to the Program Manager to ensure action is taken promptly. Work direct care shifts whenever necessary to ensure required or appropriate staffing. Be aware of the different staffing requirements between programs. This may also include medication coverage shifts or nurse‑delegated medication coverage. Evaluate and monitor program effectiveness, reporting regularly to the Supported Living Director on participant and staff needs, and implementing changes as necessary. Maintain knowledge of and interact individually with program participants and make a minimum quarterly visit to all sites. Consult with the Direct Care Supervisor and report to BPM for any concerns to report to Program Managers in the development and revision of strategies and plans for the most effective care and development. Provide instruction and support to participants on topics such as living skills, power and choice, relationship building, health and safety, and integration necessary for successful community living. Respond promptly and appropriately to challenging participant behaviors and problems at sites. Perform other duties as assigned. Education Master’s or Bachelor’s degree in social services or a closely related field from an accredited school preferred. Experience Prefer experience working with adults with developmental disabilities. Supervisory experience preferred. Licensure / Certification Agency Affiliated Counselor Registration DSHS background clearance is required, and federal fingerprinting. Obtain CPR/First Aid Certification and NAR within 90 days of hire. Become a Nurse Delegated for all clients within 6 months of becoming delegable. Complete additional clearance requirements for the Community Protection program within 6 months of hire. Current valid driver’s license and acceptable driving record. Attend and complete The Buzz in Adult Learning and Peer Coaching training within 90 days or advancement or hire. Attend training required by applicable laws. Skills / Abilities Creativity, flexibility, dependability, positive attitude, and sensitivity to diverse populations. The ability to organize work, time, and priorities. Demonstrates strong flexibility skills with the ability to exercise sound judgment in challenging, dynamic situations, including behavioral and staffing issues. Skills in reading, synthesizing, and acting upon multifaceted information. Skill with Microsoft Office products, including Word, Excel, Access, Teams, and Outlook. Typing speed of at least 40 WMP. Ability to learn Therap client database software. Strong verbal and written communication skills and demonstrated leadership ability. Perform occasional light lifting (approximately 10-20 pounds) and physically assist unsteady participants. Other Considerations $500,000 automobile liability insurance. Applied Behavior Analyst (ABA). Compensation and Benefits $26 - $31/hr. Wages determined by applicable industry experience, education, and certifications/licensure. Paid vacation - earned at .04 per hour worked for hourly employees, and 6.67 per month for salary (equates to about 2 weeks per year for full‑time employees). Sick Leave - 1 hour per 40 hours worked (equates to about 52 hours for full‑time). Holiday pay, with 8 holidays observed. Paid ‘Floating’ Holiday to be used on a day of choice.
