Visiting Speech Language Pathologist (CCC-SLP), Per Visit, Home Health - Rockville

Adventist HealthCare Home Health is seeking a Speech Language Pathologist (CCC‑SLP) servicing our Rockville Territories who will embrace our mission to extend God’s care through the ministry of physical, mental and spiritual healing. Responsibilities Perform an initial examination and evaluation to establish an impairment/diagnosis, prognosis and plan of care prior to intervention for all persons served with basic rehabilitation needs in acute care, outpatient or inpatient rehabilitation settings (flexes to any Rehab site as needed). Provide, or direct and supervise, the therapy intervention consistent with the results of the examination, evaluation, diagnosis, prognosis, and plan of care. The therapist involves the patient/client and appropriate others in the planning, implementation, and assessment of the plan of care. Reevaluate the patient/client as necessary during an episode of care to evaluate progress or change in patient/client status and modify the plan of care accordingly or discontinue speech‑language pathology services. Plan for discharge of the patient/client taking into consideration achievement of anticipated goals and expected outcomes, and provide for appropriate follow‑up or referral. Communicate, coordinate and document all aspects of patient/client management including the results of the initial examination and evaluation, diagnosis, prognosis, plan of care, interventions, response to interventions, barriers to progress, changes in patient/client status relative to the interventions, reexamination, and discharge/discontinuation of intervention and other patient/client management activities with members of the care team as indicated. Supervise and direct the patient care activities of assigned rehab aides, office assistants and volunteers. Manage schedule to accommodate range in census and patient load and inform management of any issues with caseload management. Attend team meetings and effectively communicate patient status, using, if needed, the medical record or other treating therapists to ensure sufficient knowledge of the case. Qualifications Bachelor’s, Master’s, or Doctorate in Speech‑language Pathology from an accredited program. 1‑year home health experience preferred. Valid driver’s license with agency insurance requirements. Active American Heart Association Basic Life Support (BLS) certification required. Active Maryland License. Work Schedule Employment Type: Per Visit Hours per Week: 4 hrs/week Typical Daily Schedule: 2 full days per month / weekends Shift Type: Days Weekend Requirements: As needed Holiday Requirements: As needed Pay Range $34.58 – $51.87 per hour.
