Community Outreach & Health Equity Partner
CareMore Health
CareMore Health seeks a Community Outreach and Relations Consultant to advance value-based care through strategic community engagement across assigned markets. You will build partnerships with FQHCs, faith-based groups, senior centers, and providers to improve access and equity. In this role you will analyze community health trends, coordinate with Marketing, Population Health, Care Management and Quality teams, and represent CareMore Health at events and coalitions to promote preventive care #J-18808-Ljbffr CareMore Health
