This full-time role will primarily work on-site. Successful candidates must have a valid driver's license and access to a reliable, insured vehicle. This is a grant funded role.Next Step is a hospital-based violence intervention program partnership between the City of Minneapolis and Hennepin Healthcare System (HHS). The program serves victims of violent injury (e.g. gunshot, stabbing, serious assault) who have been treated at HHS (HCMC), North Memorial, Children's Minneapolis, or Abbott-Northwestern.SUMMARYUnder general supervision, provides service to patients affected by violent crimes. Provides information, referrals and resources for clients. Assists in the care of complex patients to facilitate the delivery of cost-effective healthcare and in the identification of appropriate utilization of resources. Coaches caregivers in developmentally appropriate positive style to provide care. Refers education and outreach program activities, including implementation of appropriate policies and procedures and evaluation of program and services. Works collaboratively with interdisciplinary staff internal and external to the organization to promote health and healing.RESPONSIBILITIESExplain policies about confidential services to family members during the first visitRound on patients while they are inpatientProvide education to caregivers and survivors about conditions that may impact traumaProvide coaching to caregivers and clients for their identified supports on health issues and clientidentified goalsRefer clients to appropriate health resources and social services and support clients inaccessing resources, particularly in regards to parents/caregiversProvide positive developmentally appropriate coaching in individual and group settings under thesupervision of managementWork with survivor to develop safety planFacilitate meetings to identify appropriate visitors and or stakeholders for patients weserveProvide support on safety planning, referring emergency assistance programs and oremergency shelter or housingDevelop goal plan with assistance from clientProvide information and protect victim rights and support client with outside resources andagenciesActively refer services and follow-up with patients of high risk; work collaboratively with client andfamily who are involved in the care of the clientOrder transportation for patients to be able to utilize supports/resourcesAssist with relocation of clientsPerform face-to-face and phone contact with a specified number of patients on a scheduled basisand as neededDevelop and establish relationships with other community and social support providers to aid ineffective referrals and timely access to servicesCollect and track data for entry into the databaseParticipate in scholarly activities, presentations, train-the-trainer opportunitiesAssist clients with financial stressors under supervision of managementOther duties as assigned, but only when trained and qualified to do soQUALIFICATIONSMinimum Qualifications:Post high school educationTwo (2) years experience in program development, community health and/or grant developmentExperience working with youth/young adults and families-OR-An approved equivalent combination of education and experienceKnowledge/ Skills/ Abilities:Skill in youth/family engagement and referralBasic computer skillsKnowledge of diversity issues; grief and loss; social, economic, legal and medical systems; safetyAbility to confront discrimination, prejudice, and fearAbility to work within medical hierarchyAbility to keep accurate and timely recordsAbility to assist in complex problem-solving with clientsAbility to work autonomously, network with related agencies, build coalitions, and establish boundaries with clients, staff and agenciesAbility to be flexible and adapt to changing prioritiesMust demonstrate effective written and oral communicationAbility to deal with death and difficult issues sensitivelyAbility to verbally de-escalate hostile situationsAbility to manage personal differences for the strength of the teamLicense/Certifications:Position may require having a valid MN driver’s license and the availability of employee’s own transportationYou've made the right choice in considering Hennepin Healthcare for your employment.
