Work Environment

As a member of our team, you will work onsite in the homes of our clients, where you will provide direct physical therapy services. Our clinic operates from Monday to Friday, with working hours from 8 AM to 3 PM. The home care setting means you will engage with patients in a familiar environment, promoting comfort and facilitating effective therapy interventions. You will collaborate closely with other healthcare professionals to ensure a holistic approach to care.

Key Responsibilities

• Conduct thorough physical therapy evaluations for clients, assessing their physical abilities and needs.
• Develop, implement, and regularly update individualized treatment plans based on client assessments and progression towards therapeutic goals.
• Monitor and document treatment efficacy, using appropriate clinical measures to track patient progress and adjust strategies as necessary.
• Complete all required documentation, including SOAP notes and treatment summaries, using our electronic health record system in a timely manner.
• Educate clients and their families on the rehabilitation process, managing expectations and providing support for potential challenges.
• Meet or exceed utilization targets and contribute to the achievement of other business objectives while maintaining high-quality care standards.
• Collaborate with interdisciplinary teams to provide comprehensive care for clients requiring multiple therapy modalities.

Patient Population / Client Focus

In this role, you will interact with a diverse patient population, including individuals from various age groups, ethnic backgrounds, and with a range of physical conditions. You will provide treatment for clients with orthopedic issues, neurological conditions, post-operative recovery, and those requiring rehabilitation due to injury or chronic pain. Your capability to adapt treatment methods and nurturing an inclusive environment will be essential to support their recovery journeys.

Qualifications

• Must possess a Master's or Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program.
• Hold a current and unrestricted license to practice physical therapy in California.
• Demonstrated willingness to deliver evaluations and treatments both in-person and through telehealth as clinically appropriate.
• A strong commitment to working with a diverse range of patients with varying needs and backgrounds.
• Flexibility and adaptability in a fast-paced, ever-evolving work environment.
• Ability to work collaboratively as part of a team, with a focus on customer service and patient satisfaction.

Preferred Experience

• Prior experience in home health settings or community-based rehabilitation strongly preferred.

Compensation and Benefits
