Job Description & Requirements Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Prospect Heights, Illinois. This full‑time position supports patients in Mount Prospect and surrounding areas and is eligible for a $10,000 first‑year success bonus to reward key achievements during your first year. Responsibilities Lead and collaborate to provide management and delivery of patient care plans as a registered nurse case manager. Support patients in Mount Prospect and surrounding areas. Ensure consistent charting and meet critical deadlines for patient documentation. Qualifications RN license in the state where you work. Graduate from an approved school of practical nursing. One year of RN experience with the clinical competence to deliver quality patient care. Current driver’s license and ability to spend approximately 20% of your day driving to/from patient locations. Commitment to consistently meet critical deadlines for charting. Skill in self‑managing time and schedule. Demonstrated experience with tablets, mobile phones and EMR software.
