Job Summary:

The Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) provides essential nursing care in a long-term care or skilled nursing facility setting. This role supports patient well-being through clinical tasks, medication administration, and patient assessment.

Responsibilities:

• 
  Administer medications and treatments accurately according to physician orders.
  
  
• 
  Monitor patient conditions and promptly report changes to healthcare teams.
  
  
• 
  Document patient care activities and health status in compliance with facility policies.
  
  
• 
  Assist with patient assessments, including vital signs and physical examinations.
  
  
• 
  Maintain patient safety and comfort while adhering to infection control protocols.
  
  
• 
  Collaborate with interdisciplinary team members to coordinate patient care plans.
  
  

Qualifications:

• 
  Valid LVN or LPN license issued by the state.
  
  
• 
  Healthcare provider CPR and IV certification preferred.
  
  
• 
  Demonstrate clinical competency in a long-term or skilled nursing setting.
  
  
• 
  Ability to follow detailed instructions and document accurately.
  
  
• 
  Strong communication and interpersonal skills.
  
  
• 
  Must bring a valid ID and adhere to facility COVID-19 vaccination policies.
