Work Environment

This position is based onsite, providing home care to patients. You will be working in a flexible environment that allows for tailored interactions with patients in the comfort of their homes. Key Responsibilities

• Conduct comprehensive assessments to evaluate the health status of patients, identifying both acute and chronic medical conditions.
• Collaborate with physicians and healthcare teams to develop, implement, and adjust evidence-based treatment plans tailored to individual patient needs.
• Order and interpret necessary diagnostic tests, prescribing medications and therapies as appropriate, ensuring adherence to best practices and guidelines.
• Educate patients and their families on disease management, medication adherence, and lifestyle modifications to enhance health outcomes.
• Participate in interdisciplinary team meetings to discuss patient care strategies, fostering collaboration across various health disciplines.
• Maintain meticulous electronic medical records (EMR) with accurate documentation of assessments, treatment plans, and patient interactions.
• Engage in continuous quality improvement initiatives and apply the latest evidence-based guidelines to enhance care delivery within our team.

Patient Population / Client Focus

Your role will focus on a diverse patient population, including elderly individuals and those with complex health issues requiring in-home support. By providing personalized attention and interventions, you will empower patients to manage their health effectively and improve their quality of life.

Qualifications

• Current and valid Nurse Practitioner certification.
• Active nursing license in the state of Texas.
• Bachelor's Degree in Nursing; additional certification or master's degree preferred.
• Minimum of 2 years of clinical experience in a relevant healthcare setting.
• Proficient in utilizing electronic health record systems and other healthcare technologies.
• Possess excellent communication, problem-solving, and collaboration skills.

Preferred Experience

• Experience working in home care settings or with chronic disease management.
• Previous participation in quality improvement initiatives within a healthcare setting.
• Leadership experience or involvement in mentoring less experienced staff is preferred.

Compensation and Benefits
