Work Environment

This role is based onsite at our state-of-the-art assisted living facility in Arlington, Texas, a vibrant community known for its warmth and hospitality. You will work collaboratively with a diverse multidisciplinary team of healthcare professionals, including physicians, nurses, and support staff, all dedicated to enhancing the care experience. The work hours will typically be from 8:00 a.m. to 5:00 p.m., equating to a full-time commitment of 40 hours per week.

Key Responsibilities

As a Primary Care Nurse Practitioner, your day-to-day responsibilities will include:

• Conducting comprehensive assessments of residents to diagnose both acute and chronic medical conditions, utilizing your clinical expertise and judgment.
• Formulating personalized treatment plans by taking into account each resident's unique circumstances, preferences, and medical histories.
• Prescribing medications and therapies based on evidence-based practices, as well as ordering necessary diagnostic tests to inform clinical decisions.
• Educating residents and their families about disease prevention, management strategies, health maintenance, and lifestyle modifications to enhance their overall wellness.
• Working closely with interdisciplinary teams to ensure seamless coordination of care, including making referrals to specialists when needed.
• Maintaining accurate and up-to-date electronic health records, ensuring compliance with regulatory standards and timely documentation.
• Engaging in quality improvement initiatives aimed at enhancing patient care and adhering to established clinical guidelines.
• Staying current with advancements in healthcare practices and community health resources to optimize patient outcomes.

Patient Population / Client Focus

This position primarily focuses on assessing and managing the health of older adults residing in our assisted living facility. You will engage with a diverse patient population that includes individuals with varying levels of health complexities and chronic conditions. Qualifications

To be successful in this role, candidates must possess the following qualifications:

• A current Nurse Practitioner certification with an active state nursing license.
• A Bachelor's Degree in Nursing; a Master's Degree in Nursing is preferred.
• A minimum of 3 years of clinical experience in a primary care setting, ideally within assisted living or geriatric care.

Compensation and Benefits
