Work Environment

Your practice will take place within our outpatient office setting in Arlington, where you will be part of a collaborative care team. This exceptional team includes physicians, fellow nurse practitioners or physician assistants, medical assistants, and dedicated medical scribes, all supported by registered nurses and care managers. Key Responsibilities

• Deliver exceptional primary care services through office visits for both routine and acute health issues.
• Conduct annual wellness visits and health risk assessments that require a holistic view of patient health.
• Ensure meticulous documentation within electronic health records (EHR) to meet regulatory requirements and enhance patient care.
• Collaborate on care coordination with specialists, testing facilities, and community agencies to facilitate comprehensive patient support.
• Lead population health initiatives, such as ensuring timely screenings for conditions like breast cancer among eligible patients.
• Assist the care team with phone triage and outreach to ensure patients receive appropriate guidance and resources.
• Educate patients and their families about health conditions, proposed treatments, and care plans to promote engagement and understanding.
• Participate in promotional activities to advance community health awareness and services.
• Conduct home visits as necessary to provide care to patients with mobility challenges.
• Report directly to the Medical Director while collaborating closely with clinical and operational leadership to enhance service delivery.

Patient Population / Client Focus

The primary focus of Next Horizon Health Care is on older adults who are enrolled in Medicare. Through a patient-centered approach, our Nurse Practitioners build effective relationships that foster comprehensive care tailored to each individual's circumstances.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Possession of an active, non-probationary Nurse Practitioner license in the state of Texas.
• Current DEA license for controlled substances.
• Experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Proficient understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• Strong team-oriented mindset with a passion for collaborative healthcare delivery.
• A desire to contribute to a forward-thinking model focused on population health.

Preferred Experience

• Bilingual proficiency—particularly in languages relevant to our patient population—is highly desirable.
• Experience utilizing Electronic Health Record (EHR) systems effectively to manage patient information and documentation.

Compensation and Benefits

The annual salary for this Nurse Practitioner position ranges from USD 120,000 to 160,000, commensurate with experience and specialization. We also offer a competitive benefits package that includes:
