Remote RN Care Manager — Telehealth & Care Coordination

CareTalk Health is seeking an experienced Registered Nurse to join the virtual care team in a remote, full-time role. The RN Care Manager will manage medically complex patients, develop care plans, and coordinate AWV chart preparation across CCM, RPM, RTM, and APCM programs.
The role emphasizes telehealth workflows, independent work across multiple platforms, and documentation compliant with CMS and HIPAA standards. Evening and overnight shifts are included, with every other weekend requirements.
