Physical Therapist (PT) - General

Physical Therapist Home Health

Schedule: Monday - Friday 8 am - 5 pm

Assignment Details: Guaranteed Hours: 40 Contract Weeks: 13

Requirements:

• Current, unrestricted state Physical Therapist license
• Graduation from an accredited Physical Therapy program
• Valid driver's license and reliable transportation
• Ability to practice independently in a home-based setting
• Proficiency or willingness to learn Homecare Homebase (HCHB)

Roles & Responsibilities:

• Perform assigned physical therapy visits, including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits.
• Develop and implement individualized physical therapy plans of care based on patient needs and physician orders.
• Provide skilled interventions such as gait training, balance training, therapeutic exercise, mobility training, and fall prevention.
• Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies.
• Communicate patient progress, concerns, or changes in condition to the interdisciplinary care team.
• Collaborate in the clinical oversight of Physical Therapist Assistants (PTAs), as applicable and permitted under state law and agency policy.

Compliance / Notes:

• Documentation must comply with Medicare Conditions of Participation, payer requirements, and agency standards.
• Ensure compliance with HIPAA and all applicable privacy and security requirements.
