Key Responsibilities

• Conduct thorough patient assessments to accurately diagnose both acute and chronic medical conditions, utilizing evidence-based practices.
• Develop and implement comprehensive treatment plans, working in coordination with physicians and other allied health professionals to ensure a seamless care experience.
• Order, interpret, and analyze diagnostic tests to facilitate informed decision-making regarding patient care and manage prescribed medications and therapies.
• Educate patients and their families on disease management strategies, medication adherence, and aftercare plans to enhance recovery outcomes and prevent readmissions.
• Engage in interdisciplinary team rounds to discuss patient progress and collaboratively optimize care strategies.
• Maintain meticulous electronic medical records, ensuring accurate documentation to comply with regulatory standards and facilitate continuity of care.
• Contribute to quality improvement initiatives that adhere to best practice guidelines and enhance patient care delivery.
• Stay abreast of emerging healthcare trends, regulations, and technologies to continually enhance the quality of care delivered.

As an Advanced Nurse Practitioner, you will engage with a diverse patient population in an outpatient setting. Qualifications

• A current certification as a Nurse Practitioner.
• An active nursing license in the state of Texas.
• A Bachelor’s Degree in Nursing from an accredited institution; a Master’s degree is preferred.
• A minimum of four years of clinical experience in an outpatient or hospital setting, demonstrating a solid foundation in nursing practices.

Preferred Experience
