Description

• Conduct thorough evaluations in either inpatient or outpatient environments and craft personalized treatment plans
• Implement physical therapy interventions focused on mobility, strength, balance, and overall function
• Collaborate effectively with nurses, physicians, case managers, and fellow rehabilitation professionals to ensure the best patient outcomes
• Educate patients and their families on therapeutic objectives, discharge planning, and home exercise routines
• Document evaluations, treatments, and patient progress while adhering to facility and regulatory standards
• Engage in care team meetings and interdisciplinary discussions to maintain cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
