About the Position

Job Title: Physical Therapist – Home Health Profession: Therapy/Rehabilitation Specialty: Physical Therapy Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 year of adult home health experience required. License: Must have a valid license in hand. Certifications: CPR and BLS required. Must-Have: - Experience in home health is essential. - Must have reliable transportation and a current driver’s license. - Must have agency-required liability insurance. Description: Conduct home visits for therapy services. Participate in periodic office meetings. Average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients per day. Each visit and activity, such as documentation and travel, has a different point value. Weekly productivity expectation is 30 points. Types of cases include Start of Care and Follow-Ups. Must be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. All documentation must be completed within 24 hours of the appointment. Delayed or incomplete documentation may affect billing and patient care. Candidates must accept every patient on their schedule, regardless of distance. Weekend requirements may vary by location and profession. On-call requirements may vary; please discuss during the interview. Must be proficient in using HomeCare HomeBase EMR. No prior employees of the agency are eligible to apply as travelers. Local candidates are welcome to apply. All applicants must be open to floating to another nearby clinic if needed.
