Job Title: Temp - PT - Home Health (Days)


Job Description:
We are seeking a dedicated Physical Therapist Assistant to join our Home Health team in Kane'ohe, HI. In this role, you will plan and conduct segments of treatment programs designed to restore and improve patients' physical, social, and mental functions while aligning with department objectives. You will serve as a co-leader in an interdisciplinary approach to patient care, complete evaluations to ensure quality standards, and address assessment areas requiring attention. The ideal candidate will use appropriate clinical techniques and tools to establish individualized treatment plans, providing therapy interventions using approved protocols and techniques. You will also establish personalized care and discharge plans, ensure proper communication of these plans, and complete comprehensive clinical documentation within deadlines. Supervision of assigned assistants is part of your responsibilities, along with performing any other related duties as assigned.


Hourly Pay Rate: $58.29
