Primary Care Physician

Role: Primary Care Physician responsible for delivering equitable, value-based healthcare to Medicare patients in a neighborhood primary care setting. Responsibilities Assess, diagnose, and oversee primary care for patients Prescribe medications and treatments Maintain Electronic Medical Records using Canopy and Greenway Collaborate with regional and central leadership to meet quality goals Work with Practice Managers to guide the care team Perform additional duties as assigned Qualifications & Skills MD or DO degree; Board certification or eligibility in Internal Medicine or Family Medicine Active, unrestricted medical license and DEA registration US work authorization Preferred: Geriatrics fellowship, experience with older adults, outpatient primary care, and familiarity with value-based care models; bilingual skills are a plus Details Full-time role averaging 40 hours/week with a competitive salary range.
