School-Based Physical Therapy Contract Assignment

Minimum Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
• Current state license in good standing with the State License Board for Illinois.
• Credentials evaluated by FCCPT, ICD, ICA, IERF, or IEC when applicable.
• Experience providing physical therapy services in a school-based setting or working with pediatric populations.
• Ability to conduct assessments, develop IEPs, and maintain documentation per district, state, and federal guidelines.

Preferred Qualifications

• Experience working in self-contained or resource classroom settings at elementary and middle school levels.
• Prior experience collaborating with multidisciplinary school teams.

Assignment Details

• Part time.
• Orientation: 8/3 and 8/4.
• Resource and self-contained coverage at elementary and middle school levels.
