Position Summary

Spring Grove Recovery is seeking a compassionate, experienced, and highly motivated Nurse Practitioner (NP) to join our clinical team.

Spring Grove Recovery provides treatment services to individuals with Substance Use Disorders and co-occurring behavioral health needs. We believe effective treatment should be individualized, holistic, relational, and focused on helping each patient achieve meaningful and sustainable recovery.

The Nurse Practitioner is responsible for assessing, diagnosing, treating, and monitoring patients with medical and behavioral health needs within the practitioner's authorized scope of practice. This includes conducting comprehensive evaluations, performing history and physical examinations, ordering and reviewing appropriate diagnostic testing, prescribing medications when authorized, monitoring treatment effectiveness, and coordinating care with the interdisciplinary treatment team.

The Nurse Practitioner works closely with the Medical Director, nursing leadership, clinical staff, and other members of the treatment team to provide safe, timely, patient-centered care in accordance with applicable professional standards, facility policies, and state and federal requirements.

Essential Responsibilities

• Provide direct patient care within the Nurse Practitioner's authorized scope of practice.
• Conduct comprehensive patient assessments, including medical and behavioral health evaluations as appropriate.
• Perform medical histories and physical examinations following admission and within required facility and regulatory timeframes.
• Evaluate patients presenting with medical complaints, changes in condition, medication concerns, or other potential health issues.
• Diagnose and manage acute and chronic medical conditions within the practitioner's scope of practice.
• Prescribe, administer, modify, and discontinue medications when clinically appropriate and legally authorized.
• Order and review laboratory testing, diagnostic studies, and other clinically appropriate evaluations.
• Monitor patient response to medications and treatment interventions and make appropriate adjustments.
• Identify potential medication interactions, contraindications, adverse reactions, and other patient-safety concerns.
• Provide patient education regarding medications, medical conditions, treatment recommendations, and health maintenance.
• Provide appropriate medical support for patients experiencing withdrawal or other substance-use-related medical concerns within the services and level of care provided by the facility.
• Complete face-to-face patient assessments according to the patient's level of care, clinical needs, facility requirements, and applicable regulations.
• Conduct ongoing patient follow-up visits throughout the patient's treatment stay.
• Participate in discharge and aftercare planning as clinically appropriate.
• Make referrals or recommend transfer to a higher level of medical care when a patient's condition exceeds the facility's capabilities.
• Respond appropriately to medical emergencies and changes in patient condition.
• Maintain accurate, timely, complete, and professional documentation in the patient's medical record.
• Complete required orders, progress notes, assessments, medication documentation, and other clinical records within established timeframes.
• Protect patient confidentiality and comply with applicable privacy and confidentiality requirements.

On-Call Responsibilities

• Nurse Practitioners participate in the provider on-call rotation for their assigned region.
• Each provider may be required to take call approximately one day per week, with additional coverage when another provider is unavailable due to PTO, illness, or other unexpected circumstances.
• Calls received outside normal business hours will be routed to the designated on-call provider.
• The expected response time for on-call communications is generally within 30 minutes.
• The provider must appropriately assess the situation and determine whether telephone guidance, additional evaluation, medication intervention, emergency services, or transfer to a higher level of care is necessary.

Interdisciplinary Collaboration

• Collaborate with the Medical Director, nursing team, therapists, case managers, behavioral health staff, and other members of the interdisciplinary treatment team.
• Participate in Treatment Team meetings and other clinical meetings as required.
• Review patient progress and provide medical input regarding treatment goals and barriers to recovery.
• Communicate significant changes in patient condition to appropriate members of the treatment team.
• Coordinate with pharmacies, laboratories, hospitals, primary care providers, specialists, and other healthcare professionals when necessary for continuity of care.
• Support a professional, respectful, trauma-informed, and patient-centered treatment environment.
• Compliance & Professional Standards
• Maintain all licenses, registrations, certifications, and credentials required to perform the position.
• Practice within the Nurse Practitioner's legally authorized scope of practice.
• Comply with Spring Grove Recovery policies, procedures, clinical protocols, and professional standards.
• Comply with applicable federal and Colorado healthcare requirements and facility licensing standards.
• Comply with applicable accreditation requirements, including The Joint Commission, when applicable.
• Maintain eligibility to participate in federally funded healthcare programs when required.
• Must not be excluded, suspended, debarred, or otherwise prohibited from participation in applicable federal healthcare programs.
• Complete all required continuing education, competency assessments, and organizational training.

Immediately report patient-safety concerns, medication errors, adverse events, suspected abuse or neglect, or other reportable incidents according to facility policy and applicable requirements.

Position Requirements

Qualifications

• Master's or doctoral degree in Nursing from an accredited educational institution.
• Current and unrestricted Nurse Practitioner/Advanced Practice Registered Nurse (APRN) authority to practice in Colorado, as required for the position.
• Current prescribing authority and applicable registrations when prescribing is an essential function of the position.
• Current DEA registration, when required for assigned prescribing responsibilities.
• Current BLS certification required; additional certifications may be required based on facility services and responsibilities.
• Experience working in behavioral health, mental health, addiction medicine, detoxification, or substance use disorder treatment strongly preferred.
• Previous experience working with individuals experiencing co-occurring mental health and substance use disorders preferred.
• Strong assessment, clinical decision-making, medication-management, and documentation skills.
• Ability to recognize changes in patient condition and determine when escalation to a physician, emergency department, or higher level of care is necessary.
• Strong interpersonal and communication skills.
• Ability to work effectively as part of an interdisciplinary treatment team.
• Ability to maintain appropriate professional boundaries and patient confidentiality.
• Demonstrated commitment to compassionate, respectful, culturally responsive, and patient-centered care.

The Nurse Practitioner is expected to advocate for patient safety while maintaining appropriate clinical and professional boundaries.

Physical & Work Requirements

The position may require prolonged standing, walking, sitting, bending, and other activities associated with providing direct patient care. The Nurse Practitioner must be able to respond appropriately to urgent patient-care situations and safely perform the essential functions of the position, with reasonable accommodations provided as required by applicable law.
