Field-Based Care Manager - Senior Living & Chronic Care
The Blue Venture Fund
The Blue Venture Fund is seeking a Care Manager to provide essential support for complex and chronically ill patients within senior living communities. This full-time, field-based position will ensure a unified care plan is executed, optimizing patient outcomes while minimizing unnecessary hospitalizations. Responsibilities include developing comprehensive care plans, coordinating patient care with healthcare providers, and providing education to families about dementia and mental health.
