Role Description The Social Worker is a full-time, on-site role based in San Juan, responsible for supporting patients and families through psychosocial assessments, counseling, and care planning. Day-to-day tasks include evaluating social, emotional, and financial needs; coordinating resources and referrals; and collaborating with medical, nursing, and support staff to develop and implement individualized care plans. The Social Worker will provide crisis intervention, facilitate discharge planning, and advocate for patient rights and access to services. The role also involves documenting assessments and interventions in the medical record, participating in interdisciplinary meetings, and contributing to quality improvement and community outreach initiatives. Qualifications Demonstrated ability to perform psychosocial assessments, provide therapeutic support, and coordinate community and hospital-based resources. Strong skills in patient and family counseling, crisis intervention, and advocacy within a health care setting. Effective interpersonal communication, collaboration with interdisciplinary teams, and culturally sensitive practice with diverse populations. Solid organizational, time-management, and documentation skills, including accurate record-keeping and adherence to confidentiality standards. Master’s or Bachelor’s degree in Social Work from an accredited program; valid social work license in Puerto Rico or eligibility to obtain one. Experience in hospital or clinical social work preferred, particularly in discharge planning and community resource linkage. Proficiency in Spanish and English, with the ability to communicate clearly with patients, families, and health care professionals.
