Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate emotional, social, and financial needs of patients in our detox program.

• Provide individual and group counseling sessions that focus on anxiety, adjustment issues, grief, and coping mechanisms.

• Collaborate closely with interdisciplinary medical teams to integrate psychosocial care into treatment plans and optimize patient outcomes.

• Develop and implement personalized discharge plans, connecting patients with appropriate community resources to ensure effective transitions post-discharge.

• Deliver educational materials and emotional support to patients and their families regarding treatment options and community resources available.

• Participate in hospital committees, contributing your expertise to enhance social work practices and policies.

• Engage in crisis intervention as needed, assisting patients and their families through emotional challenges and distressing circumstances.

• Maintain meticulous documentation of all interactions, assessments, and interventions in compliance with legal and organizational standards.

In this role, you will focus primarily on individuals undergoing detoxification and recovery from substance use.
