Overview
Occupational Therapist, Acute Care — Lebanon, New Hampshire. This travel assignment begins 08/31/2026 for 13 weeks, with a guaranteed 35 hours per week. Shift: 5x8 Days, 08:00–16:30 (8.5-hour blocks). Setting: Skilled Nursing Facility. Weekly pay ranges from $2,335 to $2,410 per week. This is a critical-need assignment.

Responsibilities
- Perform comprehensive OT evaluations and develop individualized treatment plans for adults in acute rehab within a SNF setting
- Deliver evidence-based therapy to improve self-care, mobility, daily living activities, and functional independence
- Reassess progress regularly, document outcomes, and adjust treatment plans accordingly
- Collaborate with an interdisciplinary team (PT, nursing, social work) to coordinate patient goals and discharge planning
- Educate patients and families on home exercise programs, safety strategies, and equipment needs
- Ensure timely, accurate documentation and compliance with regulatory and facility policies

Requirements
- Active Occupational Therapy license in New Hampshire or eligibility for licensure
- Degree in Occupational Therapy from an ACOTE-accredited program
- 1 year of related experience in acute care or SNF settings is preferred
- Ability to work effectively in a skilled nursing facility environment and collaborate with a multidisciplinary team
- Proficiency with standard rehab documentation and outcome measures
