Patients benefit from consistent, accessible care anywhere they need it. Role Description This is a full-time, on-site Nurse Practitioner role based in Scottsdale, AZ. The Nurse Practitioner will conduct patient assessments, diagnose and treat health conditions, develop treatment plans, and provide preventative care. They will work collaboratively with physicians and other healthcare professionals to ensure comprehensive and quality care. This role will also involve patient education and active participation in a team-based care approach to improve overall patient outcomes. Qualifications Advanced clinical expertise in patient assessment, diagnosis, and treatment planning Ability to provide preventative care and educate patients on health management Collaborative skills to work effectively in a multidisciplinary team Proficiency with telehealth platforms and electronic health record systems Active Nurse Practitioner license in Arizona (or eligibility for licensure) Strong interpersonal, communication, and problem-solving skills Dedication to providing patient-centered care in a supportive environment Experience in family medicine or other specialties is a plus #J-18808-Ljbffr Synergy Health 360
