Responsibilities
- Provide direct and indirect skilled nursing care to residents under the supervision of the Registered Nurse
- Collect data on residents’ physical and psychosocial status; communicate findings to the RN and the Interdisciplinary Team
- Develop and adjust developmentally appropriate, culturally relevant plans of care with resident/family input; implement nursing diagnoses and interventions
- Reassess resident outcomes and refer to appropriate services to address all aspects of care; collaborate with the care team to meet resident needs
- Maintain professional knowledge and competency; participate in mentoring and orientation of staff; uphold safety and infection control practices
- Administer medications and monitor for adverse effects within scope of practice; document changes and report to the RN as needed

Requirements
- Current NY Licensed Practical Nurse licensure (active)
- BLS certification (AHA or RQI accepted)
- Minimum 1 year of LPN experience, preferably in a long-term care setting
- Ability to work night shifts in a geriatric long-term care environment
- Strong clinical communication and teamwork skills with residents, families, and the Interdisciplinary Team
- Demonstrated knowledge of geriatric care principles and age-specific assessment
