Community Health Worker

Job Summary:

The Community Health Worker will be responsible for the day-to-day coordination of the Southside Collaborative's health clubs and community outreach programs in a way that ensures high visibility and robust community engagement. Staff helps to build and nurture strong relationships with current and prospective partners, and leaders in San Antonio's Southside community. The Community Health Worker position will report to the Southside Collaborative's Program Manager.


Responsibilities:

• Program Management and Development:
• Organize and help manage weekly health club meetings, including logistics, activities, curriculum, and communications.
• Help to develop participant leadership in health clubs, resulting in attendees taking an increasingly active role.
• Ensure health club activities and curriculum are relevant, engaging, interactive, fun, and high-impact.
• Assist Program Manager in expanding the number of health clubs across the Southside.
• Help organize community outreach activities, including scheduling, staffing, materials, messaging, and opportunities for engagement.
• Ensure strong follow-up to community outreach activities, including communication and next steps with contacts made.
• Assist Program Manager in expanding community outreach activity across the Southside.
• Working with Community Partners:
• Help engage community partners in the Collaborative's health clubs and community outreach programs.
• Help develop and maintain a list of community partners and upcoming events.
• Help organize outreach to current and prospective community partners and upcoming events, including following up as needed.

Communications:

Communicate the objectives of Southside Collaborative to health club attendees, community outreach contacts, community members, and potential partners.

Help coordinate coalition meetings and provide regular program updates to partners.


Actively leverage web, social media, and texting to promote health club engagement and community outreach activities.


Required Qualifications:

• At least 2-years of relevant work experience in community development, education, social services, or health care.
• High school diploma/GED or college degree in related field preferred.
• Significant lived experience with community organizing around health and wellness issues.
• Community Health Worker (CHW) certification (or in process of completing).
• Familiarity with the Social Determinants of Health.
• Experience managing complex projects.
• Computer literate in MS Office suite products.
• Bilingual proficiency (English & Spanish), including speaking, writing, and reading.
• Valid Class C Texas Driver License.

Excellent oral and written communication skills. Must be an effective and open communicator with multiple audiences, including community-based organizations, all levels of government, philanthropic foundations, researchers and educators, health professionals, and service providers.


Vehicle/Licenses:

Must have daily use of a vehicle without prior notice. Must maintain current registration and automobile liability insurance that complies with Texas Law.

Ability to respond professionally in all situations.

Ability to set work priorities and evaluate and create solutions to work-related problems. Decisions should be systemic and not individually based.
