Minimum one year’s experience. Current state license as a Registered Physical Therapist / PT or currently in the process of obtaining a state license. Current CPR certification. Other requirements determined by our client facility. The therapist will establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under physician direction. Responsibilities include setting realistic and achievable goals, documenting patient conditions, and educating patients and families on appropriate physical therapy methods. Develop a plan of care for each physical therapy patient. Provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs, plan for, evaluate, and modify care to meet therapy goals. Collaborate with all disciplines to plan and evaluate team goals for each patient.
