Details Acute care hospital setting within a 20-25 bed Care Management department Covers Cardiology, Nephrology, Trauma Surgery, Ortho Surgery, Vascular Surgery, Neurosurgery, GI Surgery, Urology, Transplant, and ICU/Stepdown services Day shift schedule, 8:00am-5:00pm, 40 hours per week No weekend, holiday, or on-call requirements Uses EPIC EMR system Requirements Active RN license in North Carolina (Standard or Compact) Minimum 2 years of Care Management or Case Management experience in an acute care setting Minimum 2 years of healthcare experience as a Registered Nurse Nursing Diploma, ADN, or ASN from an accredited nursing school required; BSN or MSN preferred Strong assessment and critical thinking skills EPIC experience preferred Additional Information Coordinate care for acutely ill patients with complex medical conditions and discharge planning needs Conduct comprehensive patient assessments and document plans of care in Epic per departmental guidelines Participate in daily Care Management Touchpoints, CAPP meetings, and weekly Complex Care Meetings Collaborate with Social Workers, Utilization Managers, and Care Management Assistants to address barriers to discharge Identify and monitor high-risk and complex cases, proactively escalating concerns as needed Educate patients and families on community resources and coordinate referrals to post-acute providers Experience in an educational or teaching hospital setting is helpful Highly organized with strong attention to detail and adaptability to frequent change required American Traveler Job ID P-733477.
