Referral Coordinator

To support timely patient care, the temporary part-time Referral Coordinator will manage the intake, triage, and processing of externally referred patient care requests using Epic, ensuring accurate documentation and routing within the health system. Key responsibilities Receive, review, and process incoming referrals and related documentation from external providers Communicate with referring providers and clinic staff to obtain necessary clinical information for referral completion Identify trends and quality concerns impacting referral accuracy and report issues to leadership for operational improvement Required qualifications Bachelor's degree plus 2 years' experience in an ambulatory clinic, OR Associate's degree plus 4 years' experience, OR High School diploma plus 6 years' experience in an ambulatory clinic Knowledge of multiple ambulatory workflows Ability to interpret ICD10 diagnosis codes and clinical referral documentation Proficiency in Epic referral workflows, including referral creation and documentation management Experience managing or processing referrals across multiple clinics or services
