Job Description

General Summary:

As a Social Worker in Case Management, you will coordinate comprehensive, patient-centered care across the entire continuum of services, from admission through discharge. You will serve as a vital member of the interdisciplinary care team, ensuring seamless transitions and optimal outcomes for patients and their families.

Key Responsibilities:

 

• Develop and implement individualized care plans that integrate patient and family preferences, values, and clinical needs across the full spectrum of services
• Assess, evaluate, and address complex psychosocial, behavioral health, legal, substance abuse, and crisis-related needs with compassion and clinical expertise
• Champion patient advocacy by identifying barriers to care and mobilizing resources to ensure equitable access to services and support
• Collaborate actively with supervisors and interdisciplinary care team members to monitor plan implementation, evaluate effectiveness, and adjust strategies as needed
• Provide expert guidance on medical record documentation, complex care level determinations, and evidence-based post-discharge planning aligned with patient diagnoses and treatment goals

Qualifications

Licensure / Certification / Registration:

 

One or more of the following required:

• Counselor credential from the Michigan Board of Counseling (obtained prior to hire date or job transfer date)

 

• Social Worker credential from the Michigan Board of Social Work (any level accepted; obtained prior to hire date or job transfer date)
• Case Manager credential from the Commission for Case Manager Certification (CCMC) – preferred

Education:

• Bachelor's degree – required
• Master's degree – preferred

Experience:

• Behavioral health experience – preferred
