Care Coordinator | Spring Valley, NY • $25-$30/hour About the Role A growing homecare agency is looking for a compassionate and organized Care Coordinator to serve as the central point of contact between clients, families, and care teams. This is a people-first role where your ability to communicate clearly, stay on top of details, and juggle multiple moving parts directly impacts the quality of care clients receive every day. - Serve as the primary point of contact for clients, families, and care team members
- Develop and monitor individualized care plans to ensure client needs are being met
- Address client concerns and escalate complex cases to the appropriate team members
- Coordinate and manage caregiver schedules to ensure consistent coverage
- Track and follow up on authorizations, plan changes, and service updates
- Maintain accurate and up-to-date client records and documentation
- Liaise between clients, families, caregivers, and internal teams to ensure seamless care delivery
- Provide timely updates to all parties on care plan changes and service status
- Support intake and onboarding of new clients as needed
- Experience in a care coordination, case management, or homecare role
- Strong verbal and written communication skills
- Highly organized with excellent time management and follow-up abilities
- Compassionate, professional, and committed to client confidentiality
- Comfortable working in a fast-paced, in-office environment
