In-house Family Care Coordinator

SUMMARY FUNCTION
The In‑house Family Care Coordinator is responsible for providing support for organ donation activities within the assigned facility/facilities to maximize opportunities for organ donation. The role promotes consistency and trust in the donation process by ensuring excellent donor evaluation, management, and organ yield. It involves working closely with donor hospital personnel, physicians, and Organ Recovery Coordinators (ORCs) or Donation Coordinators (DCs) to obtain organ and tissue authorization while maintaining empathy for donor families and potential transplant recipients.RESPONSIBILITIES
Provide extensive on‑call services to obtain authorization for organ and tissue donation, with flexibility to accommodate increased donor activity or staffing shortages. Facilitate the authorization process for potential organ and tissue donor families before, during, and after death declaration. Evaluate potential donors, assess donor families, conduct medical/social history interviews, and assist families in identifying end‑of‑life decisions. Manage donors medically prior to recovery activities and direct placement of anatomical gifts as necessary. Provide transplant surgeons with information needed to determine appropriate recipients and coordinate the surgical recovery of organs and perioperative donor management. Support the Hospital Development Coordinator to identify leaders, assess roles, and collaborate on improvements in donation processes. Develop policies and procedures related to organ and tissue donation and serve as an expert clinical resource for the hospital. Visually assess donors, interpret charts, document information, and communicate findings to hospital staff and attending physicians. Advocate for donor families with hospital staff, especially in cases of neurological deterioration, cardiac cessation, or withdrawal of support. Discuss organ and tissue donation opportunities with potential donor families to obtain authorization. When a patient is no longer a potential organ donor, establish appropriate support, close the relationship with the family, and document the process. Provide bereavement program information to potential donor families and, when appropriate, provide follow‑up letters. Facilitate donor family and recipient communication in accordance with company policy. Perform other duties as assigned.PHYSICAL TRAITS
Walks, stands, and sits throughout the workday. Must be able to drive to on‑call assignments. Communicates verbally and in writing with donor families, hospital personnel, and physicians.QUALIFICATIONS
Education: RN, PA, Paramedic, or related health‑care degree or licensure; BA/BS preferred. OPO experience is required. Experience: Two to four years of healthcare experience with families, counseling, bereavement, and/or crisis intervention. Acute‑care social worker experience is strongly desired. Licenses/Certifications: Valid driver’s license with ability to pass MVR underwriting requirements. Computer Skills: Basic data entry and computer proficiency.
