RN Case Manager: Patient Flow & Discharge Planning

Ochsner Health is seeking a Registered Nurse to facilitate multidisciplinary communication and care throughout patient hospitalizations. This role requires collaboration with the care team, reviewing hospitalizations, and developing individualized discharge plans. The ideal candidate must be a graduate of an accredited nursing school with a current RN license in Missouri.
