In-Home Mental Health Case Manager — Community Support
La Frontera Center Inc.
La Frontera Center Inc. is seeking an experienced Case Manager (General Mental Health) to join EMPACT-SPC in San Tan Valley, AZ. You will provide case management and coordination services, including direct service, crisis intervention, transportation, and education to adults, children, and families. You will work within the Arizona Practice Model and 12 Principles, maintain thorough documentation, collaborate with partners, and meet weekly supervision. #J-18808-Ljbffr La Frontera Center Inc.
