Recovery Case Manager, Substance Use & Housing (Hybrid)
The People Concern
The People Concern seeks a Substance Abuse Case Manager to support individuals in permanent supportive housing. You will act as primary case manager, offering information and coordinating admissions to inpatient or intensive treatment when needed, while modeling wellness and recovery in collaboration with the HSSP team. You will work in a hybrid setting with duties spanning assessment, service linkage, advocacy, and resource coordination to promote housing stability and client well-being. #J-18808-Ljbffr The People Concern
