St. Peters Health Partners is seeking a full-time Nurse Practitioner to join our Home Based Primary Care Team, delivering high quality primary care directly to patients in the Eddy Senior Living Communities throughout the Capital Region. Eligibility requires Board Certification in Adult Gerontology or Family NP with Internal Medicine, NY license, DEA, and 2–3 years of elderly patient care experience.
