What You'll Do Complete occupational therapy evaluations, reassessments, and discharge visits Develop individualized treatment plans based on patient goals and needs Provide skilled occupational therapy services in patients' homes Assess home safety and recommend adaptive equipment and modifications Educate patients and caregivers on techniques that promote independence and safety Collaborate with physicians and interdisciplinary team members Maintain timely and accurate clinical documentation Qualifications Active Virginia Occupational Therapist License Required Valid Driver's License and Reliable Transportation Strong Communication and Documentation Skills Home Health Experience Preferred but Not Required Commitment to Delivering Exceptional Patient-Centered Care Why Human Touch Home Health?
