Minimum Qualifications

Associate degree in Physical Therapy from an accredited educational program.
Active State License is Required to Start the Assignment
BLS Certification May Be Required from AHA or ARC
PTA licensure in IL

Schedule: M-F 8:00 - 4:30
On-call or weekend requirement: No
Coverage area: Agency service area
Mileage reimbursed between visits, from first patient to last patient
