Key Responsibilities\n

• Perform comprehensive psychosocial assessments to gather pertinent information about clients' psychological and social functioning.

• Provide care coordination services to facilitate communication between multidisciplinary team members and ensure comprehensive treatment plans.

• Engage in consultation with therapy teams to discuss treatment approaches, progress, and welfare of clients.
\n
• Deliver psychotherapeutic treatment—individually, with families, or in group settings—tailored to the unique needs of each client.

• Act as a liaison with community agencies to connect clients with necessary resources and support services.
\n
• Maintain accurate documentation of treatment plans and progress notes to ensure compliance with medical records policies.

• Participate in disposition planning, advocating for clients’ needs and ensuring they receive appropriate follow-up care.

Patient Population / Client Focus\n

The Licensed Marriage and Family Therapist will primarily serve adult and geriatric populations, with plans to expand services to include children and adolescents in the future. Qualifications\n

Compensation and Benefits\n
