JOB PURPOSE: The Population Health Nurse is responsible for the implementation of Peninsula Behavioral Health's (PBH) Population Health services program with diverse job functions designed to meet specific program related requirements. This role focuses on improving the health status and care for individuals with chronic conditions: potentially complex medical, mental health, and psychosocial issues. The Population Health Nurse functions as an integral part of an interdisciplinary team, ensuring excellence in patient care, in an effort to achieve optimal clinical outcomes. Focus on assisting clients to become as independent as possible within the community and take an active role in their chronic health needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Conducts chronic condition management for individuals with target chronic conditions.
• Assesses individual needs and develops plans to address needs/issues related to conditions and/or overall health.
• Actively participates with community-based initiatives related to care management and transitions of care.
• Conducts patient education regarding condition and symptom management, red flag warning signs, the importance of adhering to medications and evidence-based guidelines and refers to supportive programs.
• Establishes self-management goal(s) with patients using self-management support concepts and motivational interviewing techniques.
• Coordinates and communicates with health plans and health care providers as appropriate.
• Apply the principles of the nursing process to care and condition management of individuals with chronic conditions.
• Assists in provider and office staff education regarding Population Health initiatives.
• Participates in screening, emergency/crisis intervention, and crisis stabilization as appropriate.
• As time permits, participates in other client services which may include home visits for the pupose of administering medication, monitoring vital signs, compliance and side effects for the medications prescribed by providers, as well as case management needs as determined by the treatment team.
• Monitors, administers, and/or follows up on prescribed medical treatment, medications, and/or laboratory test in accordance with provider orders.
• Accurately transcribes physicians' orders, maintains records, and completes documentation in accordance with Agency policy and Washington State Law.
• Provides individual and /or group education/training for patients, families, and staff.
• Although the typical workday is during normal business hours some evening and weekend hours may be required.
• Complies with Agency policies and procedures, and all applicable Federal, State and County laws and regulations.
• Maintains confidentiality of Agency and patient information.
• Maintains familiarity with and handles client information in accordance with Federal Regulations (42 CFR, Part 2), the Revised Code of Washington (RCW 71.05.390 and RCW 71.24), and other applicable laws pertaining to confidentiality of client and staff information.
• Maintains physical security of confidential materials and assigned Agency property.

SECONDARY DUTIES AND RESPONSIBILITIES:

• Provides support to the medical providers through triage, screening, processing prior authorizations and answering client phone calls.
• Participates in relevant in-service and professional training, in accordance with Agency policy and Washington State Law.
• Participates in staffing, staff meetings, and individual supervision, as assigned.
• Observes, assesses, records, and communicates patient behavior.
• Advocates for patients and coordinates services with other community agencies
• Maintains working relationships with community and government agencies, other professionals, and the family members and "significant others" of clients.
• Maintains a safe, hygienic environment for patients.
• Provides testing or ordering of tests, per standing orders, at the request from HR regarding new hires and other PBH staff.
• Performs other duties as assigned.

• Provides direct service to or on behalf of PBH clients for at least half of the hours worked annually.

REQUIRED EDUCATION, LICENSE(S), CERTIFICATION, AND EXPERIENCE:

Education: Graduation from an accredited school of nursing; valid Washington State driver's license and proof of insurance, with car available, within 30 days of employment

Licensure: Current licensure as an RN as issued by Washington State Board of Nursing

Experience: One year in clinical services with limited exceptions

Additional agency requirements:

• Must be able to pass a pre-employment drug test and background check.
• Must be able to provide vaccination records for MMR, Hep B, Tdap and recent flu shot.

PREFERRED ADDITIONAL CREDENTIALS / EXPERIENCE:

Experience: Minimum of one year experience as a staff RN in a mental health or acute care facility or skilled nursing home; two years' experience working with chronically mentally ill adults

NATURE AND SCOPE:

Physical Demands :

• Moderate physical effort (lifts / carries up to 25 lbs.)
• Frequent standing / walking.
• Occasionally lifts supplies and equipment.
• Required Protective Equipment: maintain Standard Precautions and uses barriers (gloves, masks, aprons, goggles) appropriately in all potential exposures to body fluids.
• Rarely exposed to clients with aggressive behaviors
