Work Environment\n

This remote position offers flexibility and the ability to manage your schedule while working primarily from home. You will be required to conduct on-site visits to clients in hospice settings as necessary, allowing you to monitor their conditions and provide immediate support. While your primary hours will be Monday to Friday from 8:00 AM to 4:30 PM, you should exhibit flexibility to accommodate client needs, which may involve occasional evening or weekend hours and on-call responsibilities.

Key Responsibilities\n

As a Licensed Master Social Worker, you will be responsible for the following:

• Conduct comprehensive assessments to identify the psychosocial needs of seniors, taking into account their emotional, mental, and social circumstances.

• Collaborate closely with clients and their families to craft individualized care plans that align with their unique preferences and goals.

• Provide supportive counseling and emotional guidance to seniors and their families, facilitating their navigation through life transitions such as health crises or loss.

• Advocate for clients' rights, ensuring they have access to essential community resources and services that support their well-being.

• Work as part of an interdisciplinary team, collaborating with healthcare providers, nurses, and caregivers to ensure integrated and comprehensive support for clients.

• Perform regular client visits to assess living conditions, wellbeing, and satisfaction with care services while addressing any safety issues.

• Resolve family conflicts related to the care of senior clients, promoting effective communication among all parties involved.

• Educate clients and their families about community resources, programs, and support groups that can enhance their social engagement and quality of life.

• Keep meticulous records of client interactions, assessments, care plans, and progress notes in compliance with organizational protocols.

• Stay informed on current practices, policies, and ethical standards pertaining to social work, ensuring compliance and best practices.

Patient Population / Client Focus\n

Your primary focus will be on senior clients within a hospice center, requiring cultural sensitivity, patience, and a deep understanding of the aging process and related social challenges. You will support not just the individual clients but also their families, often navigating emotional and practical hardships associated with end-of-life care.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution.

• A valid state license to practice as an LMSW.

• A minimum of two years of relevant experience in social work, particularly within community settings or geriatric care.

• Thorough understanding of social work principles, issues related to aging, and community resources.

• Strong assessment, counseling, and intervention skills tailored to the needs of seniors and their families.

Compensation and Benefits\n
