You will work closely with our hospice center, collaborating with caregivers and healthcare providers to address the unique social and emotional needs of patients during critical times. While you will primarily work from a home office, occasional meetings may be required to facilitate teamwork and client consultations.

Key Responsibilities\n

• Conduct thorough assessments of clients' emotional, social, and environmental challenges to develop meaningful intervention strategies.

• Create personalized service plans that outline specific goals and actions, ensuring ongoing progress monitoring and adjustment as needed.

• Provide empathetic individual and family counseling to support clients dealing with grief, loss, and other adjustments related to hospice care.

• Offer direct crisis intervention, including risk assessments and safety planning, particularly in urgent situations that may arise with clientele.

• Work collaboratively with other healthcare professionals, community agencies, and family members to coordinate comprehensive services for clients.

• Engage in community outreach initiatives to promote agency services, raise awareness about hospice care, and connect individuals with necessary resources.

• Attend case conferences and participate in regular team meetings to ensure integrated support for all clients.

Patient Population / Client Focus\n

Your primary focus will be on individuals and families involved with hospice care. This includes supporting clients through the complexities of end-of-life planning, managing grief, and ensuring they have access to essential resources. You will be pivotal in enhancing the psychosocial well-being of patients, fostering a supportive environment that prioritizes dignity and respect during challenging times.

Qualifications\n

• A Master's degree in Social Work from an accredited program is mandatory.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required.

Compensation and Benefits\n
