Key Responsibilities\n

As a Licensed Clinical Social Worker at Fresh Start Healthcare Services, you will:

• Conduct in-depth initial assessments of patients’ emotional and psychological needs, developing individualized treatment plans tailored to their unique circumstances.

• Utilize evidence-based practices to apply therapeutic modalities that assist patients in coping with grief, loss, and complex emotional challenges.

• Provide ongoing case management and support, adjusting treatment plans based on continuous monitoring of patient progress.

• Develop and educate staff and patients on stress management techniques and coping strategies, facilitating group and individual therapy sessions.

• Coordinate with interdisciplinary teams, community resources, and medical professionals to ensure that our patients have access to comprehensive support services.

• Document all patient interactions and interventions accurately in the Electronic Health Record (EHR) systems, meeting compliance and regulatory standards.

• Complete required continuing education courses to maintain your certification and stay abreast of the latest trends and best practices in social work and hospice care.

In this role, you will primarily serve patients who are facing terminal illnesses and their families, providing crucial mental health support and counseling. Qualifications\n

The ideal candidate for this position will possess the following qualifications:

Compensation and Benefits\n
