Relationship Behavioral Health Services is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a fully remote capacity. As a full-time LCSW, you will play a crucial role in delivering holistic counseling services within our Hospice Center, primarily focused on patients navigating the challenging terrain of co-occurring disorders and substance use. Our compensation package ranges from USD 70,000 to 105,000 yearly, commensurate with experience and qualifications.

Key Responsibilities\n

Your day-to-day responsibilities will include:

• Conducting comprehensive assessments of clients’ counseling needs, focusing particularly on those with co-occurring disorders.
\n
• Utilizing the DSM (Diagnostic and Statistical Manual) criteria for accurately diagnosing substance use and mental health conditions to determine the appropriate level of care guided by ASAM (American Society of Addiction Medicine) standards.
\n
• Drafting and executing individualized treatment plans, specifically addressing the needs related to substance abuse counseling.
\n
• Formulating discharge and continuum care plans to guarantee seamless transitions to the next appropriate levels of care.
\n
• Documenting counseling sessions, assessments, and any provided educational services in the client’s clinical record following all regulatory standards.
\n
• Maintaining an active caseload, efficiently managing treatment planning and clinical charting to reflect ongoing client engagement.
\n
• Coordinating support services for clients and their families and ensuring consistent follow-ups are recorded in the client’s clinical records.
\n
• Engaging with various referral sources, including legal, educational, and employment entities, to foster comprehensive support networks for clients.
\n
• Applying a multi-disciplinary approach during crisis situations, facilitating necessary interventions to ensure optimal patient outcomes.

Patient Population / Client Focus\n

This role will primarily focus on individuals dealing with complex co-occurring disorders, offering direct support through individual, family, and group therapy sessions. You will be an integral part of each client’s journey to recovery, utilizing evidence-based practices to promote long-term healing and resiliency.

Qualifications\n

To be considered for this position, candidates should possess:

• A Bachelor’s Degree in Social Work or a related field.

• A valid and active LCSW designation.

Preferred Experience\n
