Description

• Assess patients in both inpatient and outpatient environments, crafting customized treatment plans tailored to individual needs

• Provide education to patients and families regarding therapeutic objectives, discharge plans, and necessary home exercises
• Maintain comprehensive documentation of evaluations, treatments, and patient progress as per facility protocols
• Engage in multidisciplinary rounds and teamwork meetings to ensure seamless and integrated patient care

Education

• Earned doctorate in Physical Therapy (DPT) or related field from a recognized and accredited program

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
• Specialty certifications (neurological, geriatric, etc.) are appreciated but not mandatory

Skills

• Previous experience in a clinical setting is highly desirable
• Ability to treat patients with a variety of complex medical conditions and mobility challenges
• Strong communication and collaboration skills are essential
• Familiarity with digital charting and clinical documentation processes required
