Position Summary:
Universal Comprehensive Health Services (UCHS) is seeking a highly organized, persistent, and detail-oriented Specialty Treatment Navigation & Access Coordinator to manage the administrative progression of patients through our HIV, PrEP, and STI treatment programs.
This is not a front-desk, receptionist, scheduling, or traditional medical-office position. This is a non-clinical, workflow-focused role responsible for managing a high-volume patient treatment pipeline and ensuring patients continue progressing toward treatment and remain connected to care.
The coordinator will track assigned patients from identification and testing through required laboratory completion, clinical readiness, insurance and medication access, prescription processing, medication initiation, follow-up, refills, and retention in care.
Success in this position requires more than documenting activity. The coordinator must be able to determine what is preventing a patient from moving forward, identify who is responsible for the next step, initiate the appropriate follow-up, escalate unresolved barriers, and continue monitoring the case until the required action is completed.
We are looking for someone who follows unresolved cases through completion rather than allowing patients to remain on a tracker without a clear next action.
Treatment Pipeline Responsibilities
The coordinator will maintain active oversight of patients progressing through:
• Patient identification and initial testing
• Confirmatory and required laboratory completion
• Resolution of missing, rejected, insufficient, or outstanding laboratory work
• Completion of required clinical and administrative documentation
• Insurance and treatment-access readiness
• Provider review and prescription readiness
• Prescription and pharmacy coordination
• Medication initiation
• Follow-up and refill monitoring
• Retention in care and re-engagement of patients at risk of becoming lost to follow-up
Patient Pipeline & Case Management
• Maintain active oversight of assigned HIV, PrEP, and STI patient pipelines.
• Track each patient’s current stage, outstanding requirements, next action, responsible party, and required follow-up date.
• Identify patients who are delayed, stalled, missing requirements, overdue, or at risk of being lost to follow-up.
• Investigate why a patient has stopped progressing and initiate the appropriate follow-up.
• Continue monitoring unresolved items until they are completed, appropriately escalated, or formally resolved.
• Prioritize cases based on urgency, treatment readiness, deadlines, and length of delay.
• Ensure no patient remains on a tracking system without a documented next action and follow-up plan.
Laboratory & Clinical Requirement Tracking
• Track required laboratory orders, specimen collection, results, and outstanding clinical requirements.
• Identify missing, incomplete, rejected, insufficient, duplicate, or unresulted laboratory work.
• Coordinate with clinical staff, field teams, laboratories, correctional personnel, and other responsible parties to resolve laboratory barriers.
• Reconcile laboratory information across the EMR, laboratory portals, internal tracking systems, and other available records.
• Notify the appropriate clinical team when required information is complete and a patient is ready for clinical review.
• Escalate urgent or time-sensitive information according to established UCHS protocols.
This is a non-clinical position. The coordinator does not independently interpret laboratory results, diagnose conditions, or provide medical advice.
Treatment & Medication Access Coordination
• Track patients through insurance verification, treatment-access requirements, provider review, prescription completion, pharmacy processing, medication initiation, and refills.
• Identify missing insurance information, documentation, prescriptions, approvals, or pharmacy actions preventing treatment progression.
• Coordinate with clinical providers, insurance personnel, pharmacies, Case Managers, and other internal and external partners.
• Confirm that prescriptions and medication-related requests have reached the correct party and continue following the case until a documented outcome is received.
• Monitor medication initiation and refill deadlines and escalate patients at risk of treatment interruption.
• Maintain accurate documentation of approvals, denials, pending requirements, medication status, barriers, and next actions.
Follow-Up & Retention in Care
• Track patients who miss required laboratory work, appointments, medication starts, or refills.
• Coordinate follow-up for patients who become difficult to reach or experience barriers to continued care.
• Support continuity of care for patients transitioning between correctional, community, mobile, and outpatient settings.
• Document outreach attempts, responses, barriers, and planned next steps.
• Escalate high-risk or repeatedly unsuccessful cases according to established procedures.
• Assist with identifying and re-engaging patients at risk of becoming lost to follow-up.
Data, Systems & Documentation
• Accurately maintain patient information in eClinicalWorks, Excel-based trackers, laboratory portals, dashboards, and other UCHS systems.
• Reconcile information across multiple systems and investigate discrepancies rather than carrying incomplete or conflicting information forward.
• Maintain accurate treatment stages, case statuses, next actions, ownership assignments, and follow-up dates.
• Review records for missing or inconsistent information and obtain corrections promptly.
• Scan, upload, label, and route documentation when necessary to advance patient care.
• Maintain patient confidentiality and comply with HIPAA and organizational privacy requirements.
Performance Monitoring & Reporting
• Maintain accurate daily and weekly counts of patients within each stage of the treatment pipeline.
• Identify aging cases, overdue actions, unresolved barriers, and patients at risk of being lost to follow-up.
• Prepare concise status updates for clinical and administrative leadership.
• Identify and report recurring workflow failures, bottlenecks, and breakdowns between departments or external partners.
• Participate in routine pipeline reviews and be prepared to clearly explain each assigned patient’s current status, barrier, next action, responsible party, and follow-up date.
• Contribute to measurable performance goals involving laboratory completion, treatment readiness, medication initiation, refill completion, and retention in care.
Key Working Relationships

• Clinical providers
• Case Managers and care coordinators
• Field and outreach teams
• Correctional-facility personnel
• Insurance and patient-access staff
• Laboratories
• Pharmacies
• Administrative and operational leadership
• Patients when workflow-related outreach is required
Required Qualifications
• Minimum of 2 years of experience managing healthcare workflows, patient access, care coordination, treatment navigation, specialty medication access, clinical research coordination, population health, or another comparable high-volume patient process.
• Demonstrated experience managing a patient caseload, work queue, or multi-step healthcare process through completion.
• Experience tracking laboratory, documentation, insurance, prescription, medication, or follow-up requirements.
• Strong Microsoft Excel skills, including sorting, filtering, maintaining structured trackers, identifying overdue items, and accurately managing large patient lists.
• Experience using an EMR or electronic patient-information system.
• Exceptional organizational skills, attention to detail, and follow-through.
• Ability to work across multiple systems while maintaining accuracy.
• Strong written and verbal communication skills.
• Ability to distinguish routine responsibilities from urgent or time-sensitive barriers.
• Ability to independently prioritize a high volume of unresolved cases.
• Ability to maintain patient confidentiality and comply with HIPAA requirements.
Experience limited primarily to reception, appointment scheduling, patient check-in, or general clerical duties does not meet the experience requirements for this position.
