Key Responsibilities\n

• Conduct comprehensive clinical assessments to identify the mental, emotional, and social needs of patients, developing tailored treatment plans that adhere to best practices in hospice care.

• Employ evidence-based therapeutic interventions, including solution-focused psychotherapy, to assist patients and families in effectively coping with grief, loss, and mental health challenges.

• Maintain accurate and timely clinical records using our electronic health records (EHR) system, ensuring compliance with legal and ethical standards.

• Collaborate effectively with a multidisciplinary treatment team, including physicians, nurses, and chaplains, to ensure coordinated care and support for our patients.

• Engage in continuous professional development by staying informed on the latest research and developments in mental health practices relevant to hospice care.

Patient Population / Client Focus\n

In this role, you will work primarily with terminally ill patients and their families, providing emotional support and counseling tailored to their unique situations. Understanding the intricacies of end-of-life issues, you will help patients navigate their feelings surrounding death, loss, and bereavement, while also supporting family members during a challenging time in their lives.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
