Key Responsibilities\n

• Collaborate with a multidisciplinary healthcare team to develop personalized treatment and discharge plans based on thorough psychosocial assessments and evidence-based social work interventions.

• Conduct comprehensive assessments for patients’ emotional and behavioral concerns, identifying their needs and implementing effective strategies for support.

• Document clinical assessments, progress notes, and treatment plans meticulously according to organizational policies and standards, ensuring compliance with state and federal regulations.

• Provide crisis intervention services, counseling patients and their families through emotional challenges, thereby enhancing their coping mechanisms during difficult times.

• Consult with patients’ families and support systems to ensure an integrated approach to care and develop effective discharge plans that facilitate a smooth transition to continued support or care.

Qualifications\n
