Due to our growing patient census, we're currently looking for a Renal Licensed Master Social Worker (LMSW) to join us. Job Description As a Renal Social Worker (LMSW) , you will play a key role in supporting our patients' well-being. Your responsibilities will include: Assessing patients' psychosocial status and identifying barriers to successful treatment as part of a comprehensive assessment. Collaborating with the multidisciplinary team to develop and implement individualized care plans that improve patient outcomes and quality of life. Providing patient and family education on psychosocial issues related to End-Stage Renal Disease (ESRD) and available support services. Working with the interdisciplinary team to educate patients on all treatment options and facilitate transplant referrals. Managing the patient admission process to ensure a smooth transition and adherence to the care plan. Completing all required documentation to support the billing process. Qualifications
EDUCATION
Master's Degree in Social Work with a concentration in clinical practice from a school of Social Work accredited by the Council on Social Work Education (CSWE). Current stale licensure or appropriate certification/registration by the slate if required in order to practice social work or be called a Social Worker.EXPERIENCE AND REQUIRED SKILLS
Experience with renal patients required Excellent communication skills - verbal and written Practices continuous quality improvement Experience with helping patients deal with insurance related issues All applicants will be required to pass a criminal background check.
